Short-Term Disability Insurance

- Plan Features -

  • 60% of income up to $1,250 maximum, paid weekly
  • Payable for up to 26 weeks
  • 15 day benefit waiting period
  • 12 month pre-existing conditions limitation

Vision Insurance

- Plan Features -

  • Scheduled benefits for exams, standard lenses, frames and contact lenses
  • Use any licensed vision provider
    no required network but contracted providers are paid higher allowances, reducing the cost to the insured
  • Spouse and children coverage available.

Dental Insurance

- Plan Features -

  • Preventive Care – 100%
  • Basic Services - 80%
  • Major Services - 50%
  • $2,000 Annual Maximum
  • Orthodontia coverage for children
    lifetime maximum $1,000
  • Use any licensed dental provider
    no required network but contracted dentists agree to accept insurance as payment in full
  • Spouse and children coverage available.

Group Life / AD&D

- Plan Features -

Basic

Paid for by the employer.

  • $10,000 to $50,000 of life and matching AD&D (as selected by the employer) included for all eligible employees.

Voluntary

Additional insurance, paid for by the employee.

  • Up to $50,000 of employee voluntary life and matching AD&D as selected by the employee
  • Spouse coverage available – 50% of the employee’s amount (includes matching AD&D)
  • Child(ren) coverage available - $10,000 per child (no AD&D).
